This bug is not related to Chrome OS -- it occurs on my Pixel 3a phone.
I also filed that Chromium bug -- I wasn't sure where to file, so I filed in both Android and Chromium (not sure which side has the issue).

Description
I'm able to add the icons back by manually re-adding them, but I've had them get removed again in a few days / weeks.
I've seen both Chrome and Chrome Canary icons get removed at the same time, which made me wonder if the issue was Android-related, rather than a Chrome issue?
When the issue happened today, I believe it was during a system restart to install a Google Play system update. I think the last time the issue happened it was also after a system restart (although IIRC not for an update).
I took a bug report an hour or so after the issue occurred (I also manually added my home screen icons back before taking the bug report):
