https://android-review.googlesource.com/c/platform/frameworks/support/+/1731335 we implemented a naive keys implementation for the placeholder keys based on the index. However it will not work as expected when we prepend new items before the placeholder and when the placeholder was the first visible item.
Consider instead use not the absolute index, but the combination of the key of the nearest real item and the relative position to this item

I’ve attached two screen recordings demonstrating the mediator's behavior during the first and second launches after installation.
