Team, Suddenly Dialogflow CX End Session is not working, its not ending the session and not going back to the default start flow. Our production systems are affected

Hi Team,
We are using dialogflow CX Agent . Today we are facing issue in end session page. Once the intent got recognized it's routing into the flow and from then once we reach end session but session is not being ended.

Description
We have followed the documentation, created a knowledge base, successfully uploaded FAQs, when we try out questions from FAQ list in the ES environment panel, we are correctly directed to the FAQ location.
But once we try a Simulator within the Agent Assist, we get nothing back.
Would you please be able to guide us what we might be doing wrong?
