The api of Lazy grids span was intentionally designed to eventually support defining spans in the both directions without binary breaking changes.
We should explore possibility to add this functionality.
In the RecyclerView it wasn't possible out of the box, but it was implemented in the Nick Butcher's fork here - https://github.com/nickbutcher/plaid/blob/grid_span/app/src/main/java/io/plaidapp/ui/recyclerview/SpannedGridLayoutManager.java, and this class was even copied into the google internal repo with some usages in the Google apps.
According to Nick "Over the years a number of developers have reached out asking about it's stability so I think that there is some interest in this functionality".
The idea is to support use cases like in the attached screenshot
