Me too. I have been able to toggle BT repeatable since I got the phone a week ago, up until just now, had to boot to make it work again.
After booting, it allows me to toggle it again.

Just to give a bit more detail, both my wife and I set up google wallet also. Whenever this condition occurs (bluetooth refusing to turn on), google wallet/bluetooth will drain battery rapidly, causing battery drain of around 8%/hr when idle instead of the normal 3-4%/hr.

Same issue here also. I realized it a couple if weeks ago but now the situation has occurred more than three times. Bluetooth only works when I restart the phone. It looks like this happens if I do not restart my phone for more than two to three days. Jeff.

It turned BT on, maybe connected to my navigation system, turned it off and then when turning BT on again it almost immediatly switches off again.

Same issue here: Nexus 4, Android 4.2.1. Noticed it on the second day after the update was pushed out. No power-conserving apps installed, and no other radios were on at the time -- no GPS, no WiFi, no NFC. Sounds like a defect with Bluetooth. Only way to fix is to restart. I've had to do this twice now.

sigh, another BT/wireless problem on the N4.
N4 running 4.2.1. For whatevver reason BT't turn on in the settings, if I flip the switch to the "on" position it immediately goes back to Off.

My problem with the instantanly turning off bluetooth got fixed as it seems.
The problem started after the update to 4.2.1 as it seems. The hotline asked my to make a factory reset. I did this and after reinstalling everthing as before it works like a charm. No reboot necessary at all.
And the runtime until the next recharge is suberb now (2 days it no problem at all).

Same issue here. Unknown cause. I never use airplane mode, and disabled all apps, widgets, profiles, etc... which might manipulate bluetooth. Still happens.

Bluetooth issue is linked to WIFI. They just don't work together. Turn WIFI off, bluetooth works great, but then WIFI fails to reconnect to AP and vice versa. Other sites claim this is a driver issue.

It has happened to me three times since I had my Nexus 4 (1 month). It "fixes" when it is rebooted (and then BT appears as already on). Turning airplane mode on and off does not work.

Even though this does not seem like a hardware issue; I went ahead and got an RMA. I am testing the replacement unit right now. Will post back any finding, good or bad.
So far I tested Airplane mode on/off and Bluetooth still working afterward. Will post back more findings.

RMA unit testing day 2, no bluetooth turning off. I was able to go in and out of Airplane mode and bluetooth still working.

Just been onto Google support demanding a response to this and whether it a hardware or software issue. They told me it's the first they'd heard about it and I raised the point that how could they possibly not know about it given the number of warranty returns for this and given that I'll be on my 4th nexus from them.

Regarding my posts #44, #50. Here is my update for testing with a RMA unit. Finally after 145 hours of uptime, I tested the Bluetooth on my RMA unit:
1/ Did a test file transfer to my laptop. Success.
2/ Putting it into the airplane mode briefly and then turn off airplane mode.
3/ After that the Bluetooth remained "OFF" and there is no way to turn it back on. The indicator shows the Bluetooth is OFF before going to step 4.
4/ A power off, power on sequence will turn the Bluetooth back to ON automatically. This leads me to believe that the saved state of the Bluetooth was not touched, otherwise the after power on, the Bluetooth should have been OFF.
I draw from this test:
1/ RMA or factory reset won't help resolve the problem. These are just put the phone into a condition where it take longer to exhibit the problem.
2/ Power restart will take your device back to working Bluetooth condition. This is not a desirable remedy.
I think we all should let the press and Android team aware of this problem to bump up the priority.

Google replied to my email asking if it was a known hardware fault or know software fault. You'll notice they completely avoid answering the question about known software fault......see below.

I will try this also but this still is not a fix, just another workaround. Currently I use the "workaround" of just not turning off bluetooth. Google needs to address this problem though rather then pretending there isn't one.

I also leave Bluetooth on to avoid the issue. Unfortunately, I also run into the problem of the Nexus simply refusing to pair nearly every time I try to connect it to a device, even two devices that work fine with my Galaxy Nexus. Since I can't turn Bluetooth off and back on (a reboot lets me connect) I've found that power cycling the device I'm trying to pair with works and is faster. Utterly insane that I have to do this nearly every time just to pair a device.

Me too having similar issues when toggling the Airplane Mode ON and OFF.
To Workaround, now I turn OFF bluetooth before turning ON Airplane Mode.
Then, turn OFF Airplane mode and then turn ON bluetooth. It worked for me multiple times. To live with until it does get fixed though.. :(

Same issue even if I never turn blue tooth off. I have to restart the phone in order for the Bluetooth to connect to my car. I have to do this each and every time.

Nexus 4 Android 4.2.2.
I switch to airplane mode every night before i go to sleep (I do that to save battery because the network signal at home is very weak). In the morning, i switch back to normal mode and bluetoot does not come on. Even if i try to manually turn it on, the sliding bar just denies to slide, and bluetooth stays off. Only if I reboot, then it's all back to normal...

So, I called support and reported my Bluetooth not turning on issue. She put me on hold to talk to tech department. When she got back to me she told me that there was no problem with my phone sine i can still use it and that I just have to keep rebooting my phone every time i get that issue. I told her that have own many phones before and I have never have to reboot my phone every few days. I told her that rebooting the phone every 2-3 days is not a fix. It's a work a round that are becoming very annoying. At the end she ended up giving me an RMA.
So, after receiving my RMA after 3 days of use it happened again. This time i did not install any apps and left it all stock.

Something I'm trying to make it less annoying: if you have Tasker, program it to turn Bluetooth on and off a few times a day for a few seconds. So far it's working for me, but will report again after a few e extra days.

Google won't take responsibility for the fault. One developer implies it will be fixed in Key Lime Pie.
Now, I would happily wait 6 months for that to happen, perhaps even plumping for an N5, but judging by Googles complete inability to recognise, communicate and fix the issue, I've just ordered a Galaxy S3 for £20 more than an N4. Google have simply failed to instil faith in their competence at fixing this and this is seen among other users on these threads.
So congratulations Google. Your incompetence is now not only losing existing customers, but also potential new ones. Which is a shame because the N4 seems like a great piece of kit, save for this one minor bug (I played on a friends N4).
I'll be unsubscribing from all trackers soon. Come Thursday, I just won't give a shit about the Nexus as a brand anymore.

Same issue. I ordered my Nexus on day one. Received and I saw the problem within a few days. I ordered a replacement, and I got the same problem! I know other people with Nexus 4s and they are not seeing the issue. Could this be a software problem? I don't any weird apps installed or anything like that. I've been having this issue fairly consistently, 100% of the time. Until yesterday, when I noticed that after toggling airplane mode on and back off Bluetooth was still working. Since then I've been able to toggle Bluetooth on and off without any problems.
The only thing I did yesterday that might have affected Bluetooth was I messed around with my Nexus Q a bunch, trying to set it up at my shop. I hadn't touched it in months and I was having trouble setting it up so in the process, I deleted the Nexus Q app data, uninstalled and reinstalled it, etc. I have a hard time believing everybody here has the Nexus Q app installed but that's all I can think of...

I am running on stock 4.2.2, the latest version of JB, but I am facing a similar issue with bluetooth. my issue is not limited only to the car BT connectivity but also my MW 600 BT headset. It shows that Bluetooth is connected, but I am unable to hear the other person, i go back to handset and switch to bluetooth it works for sometime, only to repeat again. i have tried everything including only enabling phone audio, restarting the phone as well as unpair and pair the device, but to no effect. the issue continues. incidentally, my nexus is 2 months old and on the same version. the issue started around 2-3 weeks back, since then i am facing this and it is a huge problem

Posted in #165 with procedure that supposedly fixed it. However issue came back after a while. Definitely a SW issue as it's happening across phone models (Skyrocket, Nexus, etc). Google really needs to address this. Hopefully the 4.3 update will fix this.

Description
Not running any power conserving apps and Bluetooth will randomly decide to not reactivate if previously turned off. Tried putting phone into aeroplane mode and back, using widget, or directly via settings etc.
Only solution when it happens is to reboot device and Bluetooth can then be turned on again.
Ohers have also reported this and posted on Youtube.
