BTW, my new P9 has the same problem only with tap to speak, not accessibility shortcut. Happened two or three times and then stopped for some reason.
The problem on the p8 was consistent, however with each reboot.

Description
This form is only for reporting platform issues and requests related to the Android 16 Developer Preview or Beta. Only report issues that are regressions from the behavior in Android 15. For all other issues that are also reproducible on other versions of Android, please report those issues in the AOSP issue tracker at

Reboot phone, accessibility shortcut menu is on with a green shortcut on the home screen.
Toggle off in settings, and reboot. Setting is toggle on again.
