When did this happen?

What steps would let us observe this issue?
1. Click the Bluetooth button in quick settings
2. The button turns on quickly then off again
3. I was able to connect my Bluetooth headset even when the quick setting said Bluetooth was off

What did you expect to happen?
The Bluetooth quick setting being responsive and showing the right state

What actually happened?
It connected while appearing off, and didn't seem to be responsive before

How often has this happened?
Occasionally

What was the effect of this issue on your device usage, such as lost time or work?
Moderate

Additional comments
I had this issue before rebooting and it persisted after the reboot. I took the big report after the reboot.

After the connection, the quick settings tile showed the right state
